Raised by [`SignalWire.dial`](/docs/browser-sdk/v4/reference/signalwire#dial) and the inbound-call flow when a call cannot be brought to the `connecting` state — typical causes are signaling-layer failure, authentication rejection, or the server refusing the call invite.
